The following questionnaires,
assessments and test will be provided and are required to be completed in full.
|
|
|
pH Test The human body is mostly made up of water. This water-based medium
can have either acid or alkaline properties which are easily measured by a graduated scale called pH (for "potential
hydrogen"). The lower the pH number, the greater the acidity, and the higher the pH number, the greater the alkalinity.
The foods we eat today tend to create acidosis or an acidic pH. It is recommended that we stay in the higher alkaline range.
This simple test will reflect the acidity/alkalinity in your body.

This comprehensive questionnaire is
completed over a three days time. The questions cover major health concerns and intentions. It also asks for the current
dosages of vitamins and minerals, over the counter and prescription drugs. Medical and family history and general health condition
questions are asked . Included are questions regarding current state of emotions and spiritual well-being and general lifestyle.
There is a 3-Day Nutritional Analysis to be completed along with general dietary questions.

Nutri-Body
Questionnaire This
questionnaire is designed to assess bodily signs which may relate to nutritional imbalance. It sole purpose is to educate
and to inform. This analysis is often referred to as "nutritional symptomatology". It is a way of
interpreting bodily signs to find underlying causes. It is especially helpful at finding subclinical conditions that
do not show up on conventional laboratory tests.

Although widespread, candidiasis
(yeast overgrowth) is generally overlooked by the conventional medical establishment because its symptoms so closely mimic
those of other conditions. In healthy individuals with strong, functioning immune systems, it is harmless and kept in check
by good bacteria. However, if the balance of the intestinal environment is altered, Candida
proliferates, infecting other body tissues. This questionnaire's three sections of questions will help to
determine if there is a yeast connected health problem.
